Outside User Groups requesting to use MPS for single or multiple use will need to complete this form and provide proof of insurance through their insurer (required to list PWSD as anadditional insured party on their policy). Print off pages 5-10.
Outside User Groups can purchase insurance through HUB using the form is on this page. If you are purchasing insurance through HUB, a cheque for the proper amount made out to "Park West School Division" will need to be brought in as well. Print off pages 11-12.
Once the Community Use Agreement is completed and you have either the HUB Outside User Policy completed or proof of insurance through your insurer, these can be brought to MPS for processing. If you have questions, please call the MPS Main Office at 204-842-2812.
